<hello> and <capabilities> not in XSD?

The hello message doesn't seem to be part of the XML schema in draft 4. Is
there a newer version of the schema floating around?

There also does not seem to be any way to have a vendor extension to the set
of operations that can be contained in an <rpc>. Do we need an <any> in
there someplace?
