I think that the COA procedure is a way to retrieve once again location information received in a previous Access-Request (e.g. to check if the previous location is still up-to-date).

If we are talking about the civil location of the NAS, why would it have changed?

Are we talking about a NAS located on a train that moves from city to city? In most cases, we are talking about a NAS that has a static location, so that sending periodic RFC 3576 CoA-Requests purely to see if it has changed civil location makes little sense.
