You can apply for a Enterprise ID at www.iana.org under
  http://www.iana.org/protocols/forms.htm
then go to
  http://www.iana.org/cgi-bin/enterprise.pl

and fill out the form.
That is, I am assuming you do not yet have an enterprise ID.

Once you have the enterprise ID, you must decide inside your company how
to administer branches and assignments withing your Enterprise.
Once you have done that, you can assign a MIB module

   companyNameXxxMIB  MODULE-IDENTITY

   .... other info

  ::= { enterprises youEnterpriseID xx }

where xx is the number you (inside your enterprise) assigns to your MIB module.
