The latter. You can get an enterprise branhc in the OID tree.
You can check if your company/organisation already has one via:
http://www.isi.edu/in-notes/iana/assignments/enterprise-numbers
If not, then you can request for such a branch via
http://www.iana.org/cgi-bin/enterprise.pl

Yes, that is why we have such a naming structure, so that you
can extend the standards based MIB objects with extra ones of
your own (if needed).

Did you read rfc2578, rfc2579 and rfc2580?
They contain a wealth of information I think.
